Student Records Request

Avondale ESD maintains records of current and former students.  Please follow the instructions below to request student records.

Parents Requesting Student Educational Records

If you are a parent or legal guardian and in need of your child’s educational records, please complete the Request for Student Education Records Form and submit it, along with a valid form of identification to:

Educational Institutions Requesting Educational Records of Former Students

Schools requesting educational records for former Avondale ESD students should send/email an Authorization for Release of/Request for Student Records to the last school of attendance.  Email requests can be directed to the following school email addresses:
